A phantasm spell creates a mental image that usually only the caster and the subject (or subjects) of the spell can perceive. This impression is totally in the minds of the subjects. It is a personalized mental impression, all in their heads and not a fake picture or something that they actually see. Third parties viewing or studying the scene don’t notice the phantasm. All phantasms are mind-affecting spells..
The target of the phantasm must be within your maximum illusion range within line of sight and line of effect. The actual phantasm however has no limit on range, line of sight or line of effect, as long as the target can sense it.
As long as you are within range, with line of sight and line of effect, to the target to make changes to the phantasm as a standard action or as part of the action to maintain via concentration.
A target that receives an indication that the phantasm might be false, may attempt a Will saving throw against the phantasm's DC. On a success the phantasm turns to a translucent outline for that target, and loses all associated effects. Other targets are not affected in any way by this. On failure the phantasm still appears real.
A target may attempt a Perception skill check against the phantasm's saving throw DC to attempt to spot an indication that the phantasm might be false, allowing the above-mentioned saving throw.
If the target would interact with the phantasm in such a way that it might cause undesired side effects if the figment was actually real, they must make a Will saving throw against the phantasm's DC. Example's could be: running full tilt into an illusory wall, walking through illusory fire, shooting an illusory civilian, or putting your sword through an illusory expensive-looking tapestry.
